National Program Office
The University of Chicago serves as the National Program Office (NPO) for Bridging the Gap: Reducing Disparities in Diabetes Care. Dr. Marshall H. Chin and Dr. Monica E. Peek lead the work of the NPO, drawing upon their experience as clinician investigators with significant expertise in intervention development, program management, and outcomes research. The NPO collaborates with the Foundation and a network of strategic partners to promote innovative strategies to improve diabetes care among vulnerable populations, disseminate the work and findings of the initiative, and provide leadership and vision that meaningfully contributes to the national discourse on health equity.
The NPO brings together an experienced team, working in collaboration with the Foundation, to provide grantee program oversight, offer technical assistance, convene a learning collaborative among grantees, disseminate findings from the initiative, and develop strategic partnerships with regional and national stakeholders. The NPO is also responsible for developing and implementing the cross-site program evaluation to assess program implementation and outcomes across the eight grantees, including clinical outcomes, patient-reported outcomes, and cost outcomes.
